If mandatory deaths aren‘t counted for the concept of “most lethal enemy”, then the boss at the beginning of elden ring is still eligible. Unlike other souls opening bosses, I don’t think there's a single person who played the game for the first time and beat that boss, despite it being technically beatable.
There's no trick or gimmick to it at all, it's simply a boss which is tuned to be too difficult to sight-read without being able to heal, and which doesn't allow you to try fighting it again after you die. (until you have gained said healing ability and traveled across two large areas of the game.)
All that is to say: fighting that specific enemy is the literal first thing you do in that game, which means pretty much every person who ever created a character and pressed accept has almost certainly experienced at least one death there.
Of course, I would agree that The Goomba is the most likely candidate. The weight of the sheer amount of time and omnipresence of opportunity is difficult to beat. By that principle, I don't think any recently released (last 10 years) game could possibly have anything in it which cracks the top 100 of hyper-lethals.

Last night, I was randomly watching terrestrial live TV, and thinking, man, nobody watches live TV anymore. I flipped to TVK, テレビ神奈川, the local Kanagawa station, and caught the last ten minutes of some old movie called セーラー服と機関銃 (Sailor Suit and Machine Gun). Some high school girl kissed a dude’s corpse on the lips in a morgue. Then she stood on a bridge playing a harmonica. Then she walked down the street in her sailor uniform and provocative orange high heels. The movie ends with her pretending to shoot machine guns with this little boy, and she says something like 私はバカな女になりそう。きっとなる。All super long shots, no cuts. I was bummed when it ended because I thought, man, they just don’t make movies like this anymore.
Now, I’m on the train, and just listened to the last 27 minutes of this episode of the podcast, and at the end, @"exodus"#3 mentions this exact movie, seemingly for no reason??
But there must be some reason, right? Is the movie in the news? Did the director die or something? Ahhhh! Whaaat!

@“whatsarobot”#p151442 There is no reason! Unless TVK is really clued in with Barnes & Noble sales, because they had Arrow blu-rays 50% off which is what led me to finally buying this thing. but the disc came out in nov 2021 so yeah. just one of those things!
The director did die but quite a while ago.
The most likely scenario is they put it on there because the movie was massively popular at the time, and the rest is pure coincidence.
OR TVK is stealing my brain waves which I would really appreciate some residual payments for.

Anyway I’d definitely like to watch the first however many minutes of that movie there are to watch. It’s a cool one.
@“whatsarobot”#p151446 depends on the version! critics seem to recommend watching the theatrical cut first. both versions are on the blu ray and the director's cut has an additional 18 minutes or so. kind of a lot of them!
